Claims and Assistance Services
General Information

Regardless of the type of claim, whether for emergency medical expenses, a cancelled trip, lost baggage or any other benefits or assistance service included with your specific plan, you will find the appropriate claims forms and contact information here.

STEP 1

Determine the Appropriate Claim Form by identifying the name of your plan on your confirmation of coverage or certificate that was emailed to you at the time of purchase. Submitting the correct claim form to the Claim Administrator will help ensure prompt processing.

STEP 2

Complete the information and include all documents related to the type of claim you are filing. Please include a copy of your passport showing the identification page and entry/exit stamps when traveling outside the US or travel itinerary/receipts for your scheduled trip.

STEP 3

Send the completed claim form along with required documentation to the address or email listed on the form.

Medical Claim: Initial documents required include but are not limited to: all detailed bills for medical services received, receipts for payments made for medical treatment, medical documentation, doctor and hospital reports. Please review the claim form for other documents that may be required for your claim.

Trip Cancellation and Trip Interruption: Initial documents required include but are not limited to,: all receipts showing payment for the trip and proof the trip was canceled. Please review the claim form for other documents that may be required for your claim.

Medical Evacuation: All evacuation transportation expenses must be authorized in advance by the Company or its authorized representative at:

Trip Protection/Cancelation Plans Claims and Assistance Services

Trip Cancelation Plans are administered by TME Travel Insurance, Assistance Services Provided by ON CALL INTERNATIONAL and Claims Processing by CO-ORDINATED BENEFITS PLANS (CBP)

If you purchased a Trip Protection Plan underwritten by Nationwide titled TME Choice Plan, TME Preferred Plan or TME Elite Plan, please use the following claim forms and assistance service contacts and numbers.

Detailed medical statements and medical records from treating physicians

  • Copies of invoices with receipts, or applicable credit card/bank statements, for proof of payment of medical services performed and medical supplies provided
  • Please Note: A copy of the Explanation of Benefits (EOB) for any other health insurance you have may be required, in order to coordinate benefits among insurers
  • Co-ordinated Benefits Plans may follow-up with you and/or the treating physician(s)/hospital, with questions and requests for additional information as appropriate.
